Dr.B.R Ambedkar Medical Aid Scheme

The objectives of "Dr, Ambedkar Medical Aid Scheme" is to extend medical aid grants to the patients belongs to the Schpduled Castes and Scheduled Tribes suffering from serious ailments requiring surgery of Kidney, Heart, Liver, Cancer and Brain or any other life threatening -diseases includinq organ.transplant and spinal surgery, whose annual family income is not more than Rs,3,00,000/- (Rupees Three Lakhs), The Scheme will be implemented through all the States Hospitals. The scheme is offered under the Ministry of Social Justice and Empowerment's Dr.Ambedkar Foundation The scheme provides medical aid for the treatment of scheduled caste and scheduled tribe patients who are suffering from serious ailments, including surgeries of kidney, heart, liver, brain, organ transplant, spinal surgeries and of any life threating disease like cancer.

Eligibility:
The applicant shall belong to Schedule Caste and Scheduled Community.
The Annual family income shall not exceed Rs.3,00,000/- per annum.
Only those who suffering from major ailments which need surgery such as kidney, Heart, Liver, Cancer and Brain or any other life threating diseases including organ transplant and spinal surgery.

Recquired Documents:
Application form duly filled.
Original Estimated cost Certificate duly signed by the Medical Superintendent of the concerned Hospital.
Attested photocopy of the latest income certificate.
Attested photocopy of the Caste Certificate.
Attested photocopy of the Ration Card.

Documents required for Kidney transplant i.e. Relationship with beneficiary (Form 14), Details of Donor of Kidney i.e. Name, Age, Blood Group, Aadhaar Card No of the the beneficiary.

The application should be recommended and forwarded by a local MP or by the Dist,Magistrate & Collectors/Dy.Commissioner of the concerned Dist, of the Secretary in-charge of Health & Social Welfare Dept. of State/UT or members of General Body of Dr.B.R.Ambedkar Foundation on their letter head.

The duly filled in form should reach the Director, Director, Dr.B.R.Ambedkar Foundation, 15, Janpath, New Delhi, at least 15 days before the date of surgery.

Disbursement:
100% of the estimated cost of the surgery will be directly released to the concerned Hospital, with a maximum ceiling limit as mentioned below.
1). Heart Surgery - Rs. 1.25 Lakh
2). Spinal Surgery - Rs. 1.00 Lakh
3). Brain Surgery - Rs. 1.50 Lakh
4). Kidney/Organ Transplant - Rs. 3.50 Lakh
5). Kidney Surgery/Dialysis - Rs. 3.50 Lakh
6). Other life threating diseases - Rs. 1.00 Lakh
7). Cancer Surgery/Chemotherapy/Radiotherapy - Rs. 1.75 Lakh

ఫ్రెండ్స్ సోషల్ యూనియన్ డిపార్ట్మెంట్ చేత డాక్టర్ బాబాసాహెబ్ అంబేడ్కర్ ఫౌండేషన్ 15, జనపథ్, న్యూ డిల్లీ 110001 తరపున షెడ్యూల్డ్ కులం మరియు ఎస్సీ (ఎస్టీ) రోగులకు
1) గుండె శస్త్రచికిత్సకు 1.25 లక్షలు.
2) కిడ్నీ సర్జరీకి 3.5 లక్షలు.
3) క్యాన్సర్, కెమోథెరపీ, రేడియోథెరపీకి 1.75 లక్షలు.
4) మెదడు శస్త్రచికిత్సకు 1.5 లక్షలు.
5) మూత్రపిండాలు లేదా అవయవ మార్పిడి కోసం 3.5 లక్షలు.
6) వెన్నెముక శస్త్రచికిత్సకు 1.00 లక్షలు

7). ఇతర ప్రాణాంతక అనారోగ్యాలకు 1.00 లక్షలు.

ఇటువంటి ఆర్థిక సహాయం లభిస్తుంది. ఈ సహాయం కోసం

1) రోగి ఎస్సీ (ఎస్సీ) లేదా ఎస్టీ (ఎస్టీ) ఉండాలి.
2) వార్షిక ఆదాయం 3 లక్షల వరకు ఉండాలి.
దరఖాస్తు చేసేటప్పుడు కింది ధృవపత్రాలు చేర్చాలి.
1) పూర్తి దరఖాస్తు ఫారం.
2) ఆదాయ ధృవీకరణ పత్రం.
3) కుల ధృవీకరణ పత్రం.
4) రేషన్ కార్డు.
5) ఆధార్ కార్డు.
6) మెడికల్ సూపరింటెండెంట్ ధృవీకరించిన ఎస్టిమేట్ సర్టిఫికేట్ ఈ పథకాన్ని పొందటానికి డైరెక్టర్, డాక్టర్ అంబేడ్కర్ ఫౌండేషన్, 15, జనవరి మార్గం, న్యూ ఢిల్లీ -110001 మరింత సమాచారం కోసం, శస్త్రచికిత్సకు 15 రోజుల ముందు ఈ చిరునామాలో దరఖాస్తు చేసుకోవడం తప్పనిసరి
